Home Care Eligibility 

• Currently receiving or eligible for New York State Medicaid 

• Experience persistent, ongoing difficulty handling basic daily functions and/or activities

• Diagnosed with one or more chronic illness 

• At risk for continuing physical, psychosocial or functional decline 

• Determined to remain at home, in their community and able to do so safely with provided assistance 

• Seeking an alternative to Assisted Living and Nursing Home facilities
